Many projects review their risks and issues during the initiation stage and periodically afterwards but often busy project managers relegate risk and issue management down their 'to do' lists. That is until a major risk materialises as an issue.
Ongoing management of the logs, together with subsequent reviews can be managed internally or by DBTI.
